Trissie Copses Farr began her 27 year healthcare career as a Physician Assistant in direct care clinical environments. She later became involved with four different Healthcare Corporations both in the Acute Care and Long Term Care markets. Her positions included experience in sales, management, marketing, strategic planning, outsourced ancillary services management, loss prevention, risk management, business development, and regulatory compliance. She has been consulting and working with Formation Capital, LLC since 2004 and currently serves as Vice President of Formation Healthcare Group.

 

Tina Yockey RN, BSN has 31 years of experience in Long Term Care with a focus in quality management and process improvement. She has held a variety of positions including Director of Performance Improvement, Director of Nursing and In-service Coordinator and has thorough knowledge of Department of Health regulations and survey expectations for Long Term Care, Assisted Living and Adult Homes. She has been with Formation Healthcare Group as a Manager of Clinical and Regulatory Monitoring since the summer of 2007.

 

Jackie Nordhoff, RN, BSN has over 17 years experience in Long Term Care management. Prior to her Management career she practiced Pediatric and Maternal Child Health nursing in the acute care setting. The positions she has held include Regional and National Case Mix Coordinator for a 300+ facility chain based in MD; Assistant Vice President of Clinical Services for the same MD based company as well as for another small nursing home company. Additionally, she has served as a Consultant/Independent Contractor providing clinical operations support with clinical software training and education. She joined Formation Healthcare Group in June of 2008 as a Manager of Clinical and Regulatory Monitoring.

 

 Debra Rollins has a BS in Business Management and over 19 years experience with the utilization and creation of analytical databases for statistical analysis purposes.  Her experience spans across several industries including long term health care, energy and logistics.  In addition to the creation of the Formation Healthcare database, she has tracked and trended nuclear power plants, and created customized logistical solutions for one of the nation's largest transportation companies.  She has been part of the  Formation Healthcare Group since April 2006 as the Database Manager.

 

 Elizabeth Fuszner has a BA in Education with 15 years of professional experience in child education and facility development, staff development and supervision, two of those years working with a special needs organization.  Her 15 years computer experience includes five years data entry with company specific software. She has been with Formation Healthcare Group as a Data Integrity Manager since June 2007.
