Melissa Fedun

Melissa Fedun, RN, BSN is Managing Partner and co-owner of Formation Healthcare. She is an experienced nurse leader with a demonstrated history of providing clinical advisory services to identify risk exposure in the investment management industry focused on seniors housing, post-acute, and healthcare real estate. Melissa is a highly analytical registered nurse with over 20 years of experience in survey and regulatory compliance, due diligence, CCRC and SNF operational analysis, Medicare and Medicaid reimbursement systems, and quality improvement processes. She previously worked as a risk analyst at PointRight supporting the underwriting of long-term care professional liability coverage utilizing statistical models, data analysis, and clinical expertise. She is also a former Clinical Consultant with KPMG’s Senior Living Services practice, Regional Nurse Consultant, and Director of Nursing.
Melissa received a Bachelor of Science in Nursing from Waynesburg University.
Jessica Curtis

Jessica Curtis is Managing Partner and co-owner of Formation Healthcare. A recognized expert in regulatory compliance data, she has over 15 years of healthcare analytics experience including the development of Formation Healthcare’s industry-trusted skilled nursing and seniors housing risk assessment and monitoring tools PARA-SCOPE®, Quality In-Cite®, and CarePrepare™. Jessica’s background includes clinical and regulatory-focused asset management in seniors housing and long-term care, operational and financial decision support in acute care, DEA compliance, and negotiating innovative managed care agreements. Her experience spanning the continuum of care allows her to develop and enhance Formation Healthcare’s analytics and services so clients stay on the cutting-edge with insights into risk and opportunity while navigating a heavily regulated and ever-changing industry.
Jessica received a Bachelor of Science in Mathematical Sciences from Clemson University. She is a Certified Healthcare Financial Professional and Certified Revenue Cycle Associate by the Healthcare Financial Management Association, a charter member of the Intuit Quickbase Advisory Board, and an AHCA Quality Awards Senior Examiner.
Kimberly Cox
RN, IP-BC
Senior Consultant
Kimberly Cox, RN, IP-BC joined Formation’s clinical team in 2018. She has over 20 years of healthcare experience including leadership roles in seniors housing, skilled nursing, home health, and acute care. Kim has extensive experience developing policies, procedures and programs, conducting due diligence for potential acquisitions, survey and regulatory compliance monitoring, and providing oversight to teams of interdisciplinary regional personnel. She previously served as a Vice President of Clinical Services and Director of Nursing and Corporate Quality Assurance. She has also conducted presentations as a guest speaker on AOC/POC development in annual facility trainings for the state of TN.
Kim received her Associates Degree in Nursing from Asheville-Buncombe Technical Community College and is a board-certified Infection Preventionist and a geriatric diabetic certified nurse.

Angi has extensive experience providing clinical, regulatory and compliance oversight and monitoring including conducting education, presentations, policy and procedure development and revision, and exclusion monitoring. She was previously Senior Director of Compliance with Kindred Healthcare and is also a former CMS Certified Surveyor, Clinical Quality and Chief Compliance Officer, Director of Nursing, and Regional and District Nurse Consultant.
